Description: To effectively achieve the health outcomes for those you are coordinating care for, your organization must reach outside of its’ walls. We must remember that clinical care is responsible for 20 percent of a person’s health with acknowledging the remaining effects stemming from environments as well social and lifestyle factors recognizing that community partners can additionally impact these.

This one-hour session will give you best practices and strategies to identify partners as well as tips to engage them and sustain their partnerships. It will offer ways to identify those partners or collaborators from your community through recognizing Social Determinants of Health (SDOH) and community influencers. Applying the principles learned in this webinar will ultimately shift your organization toward Community Care Coordination.
