The Great Plains QIN Friday Focus 4 Health Series is intended to jump start or augment your current quality improvement efforts. Participants will connect across the Dakotas for a diverse and collaborative learning experience designed to improve patient outcomes.
During this Series, we will talk about Multi-Visit Patients and Reducing Preventable ED Visits. ‘Multi-Visit Patients’ are defined as a patient that experiences 10 or more emergency department visits within the past 12 months.
These individuals use a disproportionate amount of healthcare resources and impact readmissions, ED throughput, and length of stay. They often have complex clinical, behavioral, and social needs that require coordinated care across multiple providers and settings. Some healthcare organizations have developed strategies to improve and streamline care for MVPs by creating individualized care plans, connecting them with hospital and community resources, and tracking their progress and outcomes.
